NY XIANGHAO SWIMMING CLUB Trainers and Guardians Instructions

I grant my permission for my son/daughter to participate in the Swim Program hosted by NY XIANG HAO SWIMMING CLUB USA INC.
I (swimmer) certify that my son/daughter in good health and capable of participation in all NY XIANGHAO SWIMMING CLUB USA INC activities and practices. I understand that any kinds of sports activity have certain risks of injury. By signing this form I agree to waive all future legal liability and all financial responsibility of the NY XIANG HAO SWIMMING CLUB USA INC, and any of their respective affiliates or agents for any and all injuries or other incidents that may occur during such event effecting in any way my son/daughter.

I also, hereby, voluntarily and without compensation authorize promotional photographs of my child(ren) to be made, displayed, and provided to others of its choosing for display by them, by or on behalf of NY XIANG HAO SWIMMING CLUB USA INC during his/her participation on the swim team.

Makeup Policy: Due to the uncontrollable factors of the NY XIANGHAO SWIMMING CLUB and the venue, the course will be canceled or rescheduled, and we will arrange a unified time to make up the class. Parents are responsible for the absence of other non-club reasons. Students can refund 85% of their tuition fees from the course before the course begins. Students also can refund 80%, 60%, 40% of the tuition fee after one, two or three times of the class. However, students will not be refunded after four times of the class. The refund period will be 45 days, expiration will be considered a waiver. Students who register after the start of the course will be charged on actual times basis, but will not be replenished.

我同意我的儿子/女儿参加纽约祥豪游泳俱乐部主办的游泳项目培训。

我（游泳者）证明我(儿子/女儿)身体健康，能够参加NY XIANG HAO SWIMMING CLUB USA INC (纽约祥豪游泳俱乐部)的所有水上和陆上训练活动。我了解任何体育活动都有一定的受伤风险。签署此表格，我同意免除NY XIANG HAO SWIMMING CLUB USA INC (纽约祥豪游泳俱乐部)；及其各自的关联公司或代理商的所有未来法律责任和所有财务责任，以及在课程中可能发生的任何伤害和或其事件。

同时，本人自愿并无偿授权我的孩子在训练中和游泳活动中的照片由NY XIANG HAO SWIMMING CLUB USA INC (纽约祥豪游泳俱乐部) 制作或展示，提供给他人浏览，并同意把照片提供给NY XIANG HAO SWIMMING CLUB USA INC (纽约祥豪游泳俱乐部) 作为宣传资料。

补课说明：出于NY XIANG HAO SWIMMING CLUB USA INC (纽约祥豪游泳俱乐部) 及场地不可控制的因素导致课程取消或改期，我们会安排统一时间补课。由于其它非俱乐部原因引起的缺课一律由家长自行承担责任。学生开课前退出课程，退（学费-手续费）的85%，开课一、二、三次后退出课程，分别（学费-手续费）的80%、60%、40%。学生开课四次后不再进行退款，退款期限为45天，过期将视为放弃退款。凡阶段开课后报名的学生按次收费，但是不予以补课。
